01Problem

Patients with cranial defects need precise, personalized reconstruction

Patients with cranial defects —due to trauma, tumors, stroke or congenital deformity— need precise, personalized reconstruction. Standard solutions (methacrylate molded in the operating room, titanium mesh) do not provide an anatomical fit, involve long surgery times and show infection rates of up to 33%.

02Solution

A complete 5-stage digital workflow

A complete 5-stage digital workflow: retopology of the CT scan to extract the cranial geometry, implant design in Autodesk® Fusion™ and tooling design in Autodesk Inventor® in a coordinated workflow, 3D printing of disposable fixtures, toolpath programming in Powermill® and 5-axis CNC machining in PEEK Optima® with ±0.02 mm tolerance. Medical approval before manufacturing.

03Results

Implant delivered in 2 weeks with a perfect anatomical fit

Implant delivered in 2 weeks with a perfect anatomical fit. Operating room time reduced by more than 50%. Rejection rate below 0.1%. Preoperative surgical planning that removes improvisation in the operating room. The patient returns to normal life with a high-quality cosmetic result.

04Process

5 stages. From digital data to a certified implant.

Each step removes a source of error. The result is an implant the surgeon can place in 2.5 hours on average, with no improvisation.

01 · Data Extraction — CT Scan Retopology. The process starts with the patient's computed tomography (CT scan). The DICOM® point cloud is processed through retopology to extract the exact cranial geometry —including the bone defect and adjacent structures— producing a clean, parametric polygon mesh. This digital dataset is the foundation of the entire engineering chain: without exact data, there is no exact implant. (CT scan → Point cloud → STL mesh)

02 · CAD Design of Implant and Tooling — Autodesk Fusion + Inventor. The cranial mesh is imported into Autodesk Fusion for parametric implant design: exact fit to the bone defect, volumetric compensation for tissue loss (Natura P®) or standard anatomical reconstruction (Natura S®), asymmetry correction and design of the microplate anchors. The machining-specific workholding fixtures are designed on the same platform, eliminating the need for additional software or processes. (Autodesk Fusion · product design + Autodesk Inventor · tooling design, in a coordinated workflow)

03 · Tooling Fabrication by 3D Printing — Disposable Fixtures. The fixtures and tooling designed in Inventor are produced directly by additive manufacturing (FDM). Because they are single-use, disposable parts, the cost and lead time of conventional metal tooling are eliminated. Each fixture is optimized for the specific geometry of each patient's implant, ensuring perfect workholding during machining and preventing vibration or shifting that would compromise the final tolerance. (3D printing · Disposable fixtures · Zero tooling cost)

04 · 5-Axis CNC Programming — Toolpaths in Powermill. The implant CAD model is imported into Autodesk Powermill to generate the 5-axis machining strategies. Roughing, semi-finishing and finishing toolpaths are calculated, optimizing tool orientation to reach every complex surface of the skull —including undercut zones— without repositioning. Virtual simulation in Powermill confirms there are no collisions between tool, holder and fixture before the program runs on the machine. (Powermill · 5 axes · Collision simulation)

05 · 5-Axis CNC Machining in PEEK Optima® — ±0.02 mm Tolerance. The implant is machined from a block of PEEK Optima® (certified medical grade) on a 5-axis CNC mill. Simultaneous 5-axis capability reaches all of the skull's organic geometry in a single setup, holding ±0.02 mm across the entire surface. The result is an implant that fits the patient's bone defect with sub-millimeter precision, reducing fitting time in the operating room to practically zero. (5-axis CNC · Medical-grade PEEK Optima® · ±0.02 mm)

05Technology stack

Five tools. One implant with no margin for error.

Each tool was selected to eliminate a source of variability. Together, they ensure ±0.02 mm in an aerospace-grade material applied to medicine.

Data acquisition — CT Scan + Retopology. Computed tomography provides the exact geometry of the cranial defect in DICOM format. Retopology turns the point cloud into a clean parametric mesh, ready for CAD design. No physical contact with the patient. No molds. No distortion.

Design · Engineering · Tooling — Autodesk Fusion + Inventor. A coordinated workflow for the three design processes: implant geometry, volumetric compensation and machining fixtures. One platform, zero data transfers, zero conversion errors. The physician approves the virtual design before a single chip is cut.

Tooling manufacturing — 3D printing, fixtures. Workholding fixtures are 3D printed on demand, specific to each implant. Disposable by design: they eliminate the cost and lead time of conventional metal tooling. Each patient has their own fixture, ensuring perfect workholding during machining.

CNC programming — Autodesk Powermill. Toolpaths optimized for simultaneous 5-axis machining. Full anti-collision simulation before execution. Roughing, semi-finishing and finishing strategies calibrated for the properties of PEEK: a hard, abrasive material with low thermal conductivity.

Final manufacturing — 5-axis CNC · PEEK Optima®. Machined on a simultaneous 5-axis CNC mill from a block of medical-grade PEEK Optima®. Guaranteed tolerance of ±0.02 mm. 5-axis capability to reach every organic surface of the skull in a single setup, with no repositioning errors.

Certified material — Medical-grade PEEK Optima®. Medical-grade polyether ether ketone certified by Invibio®. 99.9% biocompatibility, mechanical strength identical to cortical bone, full radiolucency (does not interfere with postoperative imaging), 0% water absorption, and an infection rate below 0.1%.

06In the operating room

Preoperative planning. Predictable surgery.

A digitally designed implant removes improvised decisions in the operating room. The surgeon knows the exact approach before entering the OR, reducing surgery time by more than 50% compared with traditional techniques.

  • Average placement time: ~2.5 hours
  • Compatible with titanium microplates, CranioFix® and self-drilling screws
  • Infection rate <0.1% (vs 33% for methacrylate)
  • Radiolucent: does not interfere with postoperative CT/MRI
  • In case of infection, the implant can be reused after sterilization

07Comparison

Why PEEK. Why now.

Traditional alternatives shift the cost to the patient in the form of repeat surgeries, infections and suboptimal outcomes.

High risk — Methacrylate: 33% infection and rejection rate; molded by hand in the operating room; absorbs water, allowing bacterial growth; brittle, breaks on impact; no anatomical fit or volumetric compensation; surgery time up to 4× longer.

Medium risk — Titanium mesh: tends to become embedded in soft tissue; deforms and can become exposed through the skin; does not withstand direct impacts; requires prolonged care and follow-up; unaesthetic, with visible deformation; may require a third corrective surgery.

Definitive solution — PEEK Optima® — AxonID: <0.1% infection rate; ±0.02 mm manufacturing tolerance; mechanical strength identical to bone; preoperative surgical planning; delivery in 2 weeks vs 6 months for titanium; COFEPRIS Sanitary Registration.

08Certification and regulation

Engineering that operates within the regulatory framework.

A medical implant without regulatory approval cannot be implanted. Every PEEK implant we manufacture operates under an active sanitary registration.
